Why does my provider show as 'unknown' instead of in-network?

"Unknown" means the national provider database didn't have complete data to confirm the match—it doesn't necessarily mean the provider is out-of-network. Incomplete addresses, multiple office locations, or missing provider records can all cause an "unknown" status.

To confirm network status, use your carrier's provider search tool or call the provider's office directly and ask about the specific plan you're considering.
